Laptop Loan Request

Please read the following before submitting a laptop request (the request form can be found below):

 

  1. You should inspect all equipment for any defects or other objections to the equipment before acceptance. If you accept delivery of the equipment, you are presumed to have accepted the equipment in good condition and repair.

 

  1. No other individual or entity may use the equipment for the entire time in which the equipment is on loan to you. Equipment is not to be used for anything other than its intended purpose during the loan.

 

  1. Molloy reserves the right to immediately terminate this loan and confiscate the equipment at any time.

 

  1. Great care must be taken in preserving the equipment in good condition and any damage to said equipment will be reported in writing to Molloy University Information Commons (Public Square, 2nd Floor) or Technology Support Services (Kellenberg 22) within 48 hours of occurrence. You shall not attempt to make any repairs without the express permission of the Molloy University Information Commons or Technology Support Services.

 

  1. The equipment is Molloy property; therefore, Molloy may examine the equipment and search their contents at any time for any reason. You have no right to privacy of any data saved on the equipment or in a cloud-based account to which the equipment connects. Any files saved to the equipment will be deleted upon return.

 

  1. Your signature on this form represents your acceptance to return the equipment on the date above. You understand that the equipment must be returned to the Information Commons (Public Square, 2nd Floor) or Technology Support Services (Kellenberg 22).

 

  1. You assume financial responsibility for lost or damaged equipment. If the equipment is not returned by the date above, returned in damaged condition, or lost, the student’s account will show an outstanding balance resulting in the charge for the equipment. The decision to assess a charge, as well as the amount of any charge, is at the sole discretion of Molloy, but will not be greater than the full replacement value of the equipment. It may also result in a referral to Student Affairs for review through the student conduct process.

 

  1. You acknowledge that you have read, understand, and agree to follow all responsibilities outlined in this form and agree to be bound by it. You also agree that by signing this agreement you waive any and all claims you (and your heirs, successors, and assigns) may have against Molloy University, its Board members, employees, and agents from any and all claims, damages, losses, causes of action, and the like relating to, connected with, or arising from the use of the equipment or from this agreement.

 

  1. To the fullest extent permitted by law, you agree to indemnify, defend, and hold harmless Molloy University, its individual Board members, employees, and agents, from any and all claims, damages, losses, causes of action, and the like relating to, connected with, or arising from the use of the equipment or from this Agreement.
